PATIENT POLICIES & PROCEDURES
Payment Methods: While we accept many forms of payment including Visa and Apple Pay, we prefer cashless transactions at our clinic. Please note, we do not accept checks and cannot accept insurance at this time.
Late Arrival/Cancellations: The CCOM Clinic has a 24-hour cancellation policy. While we do our best to accommodate our patients’ schedules, we cannot guarantee you will be able to receive a treatment if arriving more than 15 minutes into your appointment time.
As of January 1, 2024, patients will receive two late cancellations annually without a penalty fee. For any further late cancellations, patients will be charged the full price of their scheduled treatment.
Patients who accumulate multiple late cancellations or no-shows may be asked to schedule only same-day appointments. We thank you for your understanding.
Discounts: We offer discounts for seniors (over 60 years of age), students with a valid/current ID, military personnel, and veterans, for acupuncture treatments or herbal consults done by interns or licensed supervisors.
Eating and Drinking: We ask that our patients refrain from eating and drinking in our waiting area.
Children: Children ages 18 and under must be accompanied by an adult at all times. We do not treat children younger than 13 years of age.
Pets: No pets are allowed at the Clinic except for service dogs.
Herbal Pharmacy: We are pleased to offer herbal formulas from our in-house dispensary in raw and pill form. The Millennium III Clinic has one of the few fully stocked herbal pharmacies in the downtown Chicago area. The clinic does not accept returns on raw or bottled herbal formulas, including sealed packages of herbs and the patient agrees to the full cost of the formula. This policy is in keeping with industry standards and legal guidelines.
